The Problem Is Bigger Than Most Executives Acknowledge
When most executives think about insider threats, the mental image is the disgruntled employee walking out with a USB drive. That picture is incomplete in ways that create serious exposure.
Insider threats come in three categories: malicious insiders acting with intent, negligent insiders whose careless behavior creates the opening, and compromised insiders whose credentials have been taken over by an external actor who now operates with legitimate access. All three categories represent an insider threat problem. All three require different detection approaches. And all three are underrepresented in most financial institution security programs.
The numbers are sobering. According to the 2025 Ponemon Institute Cost of Insider Risks Global Report, the average annual cost of insider threat incidents has reached $17.4 million, up 40% over four years. For financial services specifically, the exposure is compounded by the density of high-value targets, complex legacy permission structures accumulated over decades, and the depth of third-party access that most institutions have never fully audited. Verizon’s 2024 Data Breach Investigations Report found that more than two-thirds of breaches included a human element, encompassing insider errors, compromised credentials, and social engineering, and that figure has remained consistent year over year. When you account for all three insider threat categories, not just the malicious actor, the exposure picture for financial institutions gets significantly larger.
The gap that should concern executives isn’t whether their institution has an insider threat program. Most do. The gap is between having a program and having one that would actually detect a real incident before material damage occurs.
Where Traditional Detection Falls Short
Most insider threat programs in financial institutions were designed around two things: satisfying examiners and catching obvious violations. Both are reasonable objectives. Neither is sufficient.
Data Loss Prevention tools, deployed at nearly every regulated institution, generate enormous volumes of alerts built around keyword triggers and file type rules. In practice, they catch negligent behavior with some reliability. A compliance officer accidentally emailing a file to a personal account is the kind of thing these systems find. A motivated insider who understands the environment and takes their time is a different problem entirely.
The SIEM logs everything. But logging is not detection. Security information and event management platforms are calibrated, in most environments, around rules written to satisfy compliance frameworks. The rules are written to demonstrate that monitoring is in place, not necessarily to surface behavioral anomalies in real time. The result is a system that produces reports regulators can review while a patient, credentialed insider operates below the detection threshold.
The executive blind spot here is a natural one. When the CISO reports that monitoring tools are deployed and logging is comprehensive, that’s true. It doesn’t mean the institution would catch someone with legitimate access doing something they shouldn’t. Those are different questions.
Where AI Tools Are Actually Making a Difference
This is an area where the technology has genuinely matured, though the vendor conversation around it requires some healthy skepticism.
User and Entity Behavior Analytics, particularly platforms using machine learning to establish behavioral baselines, represent a meaningful step forward over rule-based monitoring. The underlying logic is sound: instead of writing rules for every possible bad behavior, establish what normal looks like for a given user, role, and system, then flag statistically significant deviations. An analyst who accesses the same core systems every day suddenly pulling large amounts of data from a system outside their normal pattern is a signal worth investigating, even if no policy rule was technically violated.
Where the vendor conversation gets slippery is in the baselining requirements. Effective behavioral analytics require clean, well-structured access data and enough time to actually learn normal patterns before they’re useful. Institutions that deploy these tools expecting immediate detection capability are often disappointed. The technology works, but it requires operational discipline underneath it to produce reliable signal.
AI-assisted investigation workflows are a more immediately practical application. The analyst workload problem in security operations is real, and using AI tools to help triage alerts, surface relevant context, and prioritize investigation queues is producing genuine efficiency gains. The key distinction executives should understand is that this is about improving analyst throughput and judgment, not replacing analyst judgment. An AI layer that summarizes why an alert was flagged and pulls relevant user history is valuable. An AI layer making autonomous decisions about threat validity without human review introduces a different category of risk.
One honest caveat for any institution evaluating AI-powered detection: the tool is only as good as the access architecture and data hygiene sitting underneath it. Deploying sophisticated behavioral analytics on top of a permission model where users have accumulated access rights over years without systematic review will produce noise, not signal. The technology amplifies what’s already there. If what’s already there is a mess, that’s what gets amplified.
The Privileged User Problem
This is the insider threat scenario that should concern executives most, and it receives the least attention in standard detection programs.
Administrators, developers, and power users with elevated access have both the access and the technical knowledge to operate in ways that are genuinely difficult to detect. They understand what normal looks like in the environment. They know what generates alerts. They often have the ability to modify logs or access configurations in ways that standard users don’t.
This is a governance and architecture problem before it’s a technology problem. The right questions are structural: Who has privileged access? How was that access granted and when was it last reviewed? What controls exist on what privileged users can do with that access? Can a single privileged account cause material damage, or are there architectural controls that limit blast radius?
From a technical standpoint, controls like just-in-time privileged access, where elevated permissions are granted for specific tasks and time windows rather than held permanently, meaningfully reduce exposure. Session recording for privileged access provides forensic capability even when real-time detection fails. These aren’t AI-native capabilities, but they’re foundational to any detection architecture that takes the privileged user problem seriously.
What a Credible Detection Program Actually Requires
For executives evaluating their insider threat posture, a few components separate programs that work from programs that look good on paper.
The first is definitional: you cannot detect deviation from normal until you have actually defined what normal looks like in your specific environment. This is harder than it sounds. Access patterns at a regional bank reflect years of system evolution, role changes, and accumulated permissions. Establishing a credible baseline requires investment, not just tool deployment.
Deception technology, specifically honeytokens and canary credentials placed in high-value areas, offers something most detection tools don’t: a high-confidence, low-noise signal. A real user operating within their legitimate job function will never touch a honeytoken. When something does, you know immediately that something is wrong. AI is improving how these assets are deployed and monitored, making it more practical to maintain them at scale across complex environments.
Red team exercises that specifically test insider threat detection, rather than just perimeter defenses, are underutilized in financial services. An external penetration test tells you something about your perimeter. It tells you very little about whether your monitoring would catch a credentialed insider moving laterally through your environment. These are different tests and they require different methodologies.
Finally, there is a feedback loop that high-performing security programs maintain and most don’t: using detection findings to inform access architecture. Every anomaly that gets investigated, every near-miss, every red team finding that exposed a detection gap should feed back into decisions about how access is structured, provisioned, and reviewed. Detection isn’t a static deployment. It’s an ongoing calibration effort against your specific environment, and the institutions that treat it that way are materially harder targets.
The Right Question to Be Asking
For any executive evaluating where their institution stands, the question isn’t “do we have tools deployed for this?” Almost every institution does. The question is: “Would we actually catch someone with legitimate access doing something they shouldn’t?”
AI is improving the honest answer to that question. But only in organizations that have done the foundational work first. The technology is genuinely useful. It is not a substitute for the harder, less glamorous work of understanding your access model, cleaning up your data, and building detection programs calibrated to real behavior rather than regulatory checkboxes.
